This Food Service Operation has undergone a final pre-licensing inspection and has been approved to operate. A signed application and payment for the amount of $481.00 shall be submitted the City of Kent Health department. License will be issued once signed application and payment is received. Final Building and Fire approval was received at time of inspection. See building approval form in plan review folder.

Okay to operate!

Notes: These items shall be completed before the beginning of the next licensing year (at least before March 1, 2014).
1) Provide a smooth, durable and easily cleanable surface behind cold and hold holding line. Dry wall is already showing signs of damage. 2) Provide a swing door to kitchen area that remains closed at all times. Door shall be provided with a window and a "employees only" sign. 3) Lights over the main food preparation line shall be shatter proof. Provide documentation of light bulbs that are used. 4) Front windows are not to be used until screens are installed properly. 5) Provide a sneeze guard on the left side of the hot holding line and "rope" the right side of main front. 6) Provide rest-room doors to be self-closing. 7) Provide final Certificate of Occupancy at 30 day inspection. 8) Provide a Date of Completion letter for all issues that are to be corrected.
