Corrected During Inspection Multiple single-service and single-use articles not protected from contamination in the back dry storage area. PIC covered unprotected single-use items.

3717-1-04.3(C) / Fixed equipment installation: elevation or sealing.
Equipment not properly elevated or sealed to floor. Observed shelving racks with the bottom shelves installed directly on the floor in the walk-in freezer.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment with an accumulation of dust, dirt, food residue, and other debris for the following equiment: 1) Shelving in walk-in refrigeration and freezer (clear liners, carboard liners shall be removed). 2) Walls in the walk-in refrigeration unit. 3) Reach-in freezer under the front service counter. 4) Single-use (cups, bowls, spoons, etc.) container or dispensing units.

Observed the plumbing system near the water filters in the dish room and the plumbing fixture for the main hand sink leaking water and not maintained.

3717-1-05.3(I) / Other liquid wastes and rainwater.
Repeat Improper draining of condensation and other nonsewage liquids for the walk-in cold holding unit and for the ice bin next to the salad prep cold holding unit (not in use). Condensation lines were missing and drain into buckets.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
The floors in the following locations not maintained clean: 1) under and around the smoothie refrigeration unit. 2) under the equipment on the cook line. 3) Under shelving in the walk-in freezer.

3717-1-06.1(A) / Floors, walls, and ceilings.
Walls and ceilings not smooth and easily cleanable throughout the facility. Observed unfinished surfaces and missing ceiling tiles within the facility.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed damaged paint and surfaces on the exterior of the walk-in refrigeration unit and walk-in freezer.

3717-1-06.1(I) / Light bulbs - protective shielding.
No protective shielding on light above the cake decorating table near the dish room. A new light fixture is scheduled to replace the current fixture above the cake decorating table.

Ensure frozen smoothie mixes are being date marked as specified on packaging.
A plumbing system shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the FSO (ice machine, coffee machine, slushy machine, etc.).
The can opener shall be cleaned as often as needed to be clean to sight and touch.
The ice maker components shall be replaced when damaged or cracking begins.
Personal belongings shall be kept in designated locations away from the operations products.

This facility has a limitation on its license: Operation has not been provided with a properly installed Food Processing sink. All food shall be properly washed, prepared, and provided with a date of preparation at the supplier before being delivered to this location.

The violations documented shall be corrected and will be checked during next standard inspection.
