Comments
In order for a FSO to be approved to operate, the following must be completed:
1) Provide rest-rooms with self closing doors
2) Provide a swing door with window to kitchen. This door must remain shut all all times of operation. Provide a sign stating "employees only".
3) Provide a copy Person-In-Charge training (Level One).
4) Provide splash guard on right side of food preparation area hand washing sink (back hand sink).
5) Provide soap, paper towels, hand washing signs "Employees Must Wash Hands", and trash cans at all hand washing stations.
6) Provide a mop hanger in mop room.
7) Provide FRP (a smooth, durable, and easily cleanable surface at the following location: Behind the line coolers, right side of women's rest-room 18 inches from source of splash.
8) Edge finish trim is required on main food service line.
9) Three light over main service line must be shatter-proof. Provide shatter proof lighting or shields on lights.
10) If windows are to be opened screens must be installed or air curtains.
11) The following locations shall be caulked, coved or sealed to allow for easy cleaning: Caulk all gaps in tiles/walls/door frames/etc. junctures, Caulk around all plumbing fixture (base of mops sink, all hand washing sinks, back of 3 Compartment sink), Caulk and fill hole where grease trap drainage pipe is entering wall, and any other location where cove base, caulking and sealing is needed.
12) Signed and completed application and payment of license fee for the amount of $481.00.
13) All coolers must be on and holding at 38 degrees F. Cold food must hold had 41 Degrees F or below and hot foods must hold at 135 degree F or above.
