[?] A summary of the violations found during the inspection are listed below.

3717-1-03.1(R)(1) / Shellstock - maintaining identification.
Critical Shellstock tags were removed from the container. Observed the FSO without proper identification for shellfish tags at time of inspection.
To ensure proper identification, shellstock tags or labels shall remain attached to the container in which shellstock are received until the container is empty. Identification shall be kept for at least 90 days.

3717-1-02.4(A)(2) / Person in charge: assignment of responsibility.
Observed facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection at time of inspection.
To prevent health hazards, obtain the ODH level 2 certification in food protection. At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Repeat Equipment and/or components are not maintained in good working order. Observed multiple coolers in need of repair at time of inspection (broken lids, damaged handles, and interior leaks).
To prevent health hazards, repair the coolers. Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
3717-1-04.8(A) / Equipment and utensils - air-drying required.
Equipment and utensils are not being air dried. Observed drinking glasses stacked together wet without being air dried at time of inspection.
To promote proper sanitization, air dry glasses before stacking. After cleaning and sanitizing, equipment and utensils shall be air-dried or used after adequate draining before contact with food and utensils, and may not be cloth dried.
3717-1-05.4(B) / Outdoor storage surface.
Repeat Observed outdoor refuse storage surface is not properly constructed, and sloped to drain at time of inspection.
To prevent health hazards, an outdoor storage surface for refuse, recyclables, or returnables shall be constructed of nonabsorbent material and shall be smooth, durable, and sloped to drain.
3717-1-06.4(A) / Repairing.
The physical facilities are not being maintained in good repair. Observed a water damaged ceiling tile above the ware wash machine at time of inspection.
To prevent health hazards, replace the damaged ceiling tile and ensure that the leaking roof has been repaired. The physical facilities shall be maintained in good repair.
