3717-1-06.2(C) / Handwashing sinks - hand drying provision.
Corrected During Inspection No towels or drying device at the handwashing sink. PIC obtained papaer towels and placed them at the sink.

3717-1-03.2(K) / In-use utensils - between-use storage.
Repeat In-use utensils improperly stored. Observed utensils being stored in dirty water. Utensils that are going to be stored in water shall have a temperature above 135 degrees F.

3717-1-04.8(E)(3) / Single-service articles and single-use articles - storage.
Single-service and single-use articles not protected from contamination. Observed products (drink holders, paper products, etc.) stored on the floor behind the service window in the main prep area and throughout the facility.

3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Repeat Non-food contact surfaces not cleaned at the required frequency. Observed exterior of product containers, shelving throughout, items in storage, handwashing sink, three compartment sink, waffle makers and other equipment that is no longer used,etc. with an accumulation with food residue and debris.

3717-1-05.4(K) / Storing refuse, recyclables, and returnables.
Waste receptacles for refuse or recyclables stored so they are accessible to insects and/or rodents. Observed trash on the floor next to the overflowing waste receptacle in prep area.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Unnecessary or nonfunctional items on premises. Observed multiple equipment that is no longer used that shall be removed. Observed personal belongings stored throughout the facility, items shall be kep in designated location away from the operation's products.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Observed the floor in the service and preparation area with accumulation of debris, food residue, and dirt along the perimeter and under equipment. Items throughout shall be stored 6 inches off the floor to allow for easy cleaning. Facility not maintained clean.

Repeat Physical facilities not maintained in good repair. Observed damaged and stained ceiling tiles in the bathroom.
