[?] A summary of the violations found during the inspection are listed below.

3717-1-06.4(K) / Controlling pests.
Critical Repeat Presence of insects. Cock roaches were observed on pest monitoring stations in the kitchen.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Non-food contact surfaces of equipment are unclean. Multiple pieces of equipment are not clean in the kitchen (cold holding equipment, freezer, cooking equipment, exhaust hoods, sinks, tables, storage racks and shelving).

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Unnecessary or nonfunctional items stored in the basement (old equipment and excess clutter).

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Floors throughout the kitchen are not maintained clean in the hard to reach areas under/behind equipment.

The following violation(s) have been corrected since the last inspection.
Comments
The FSO was observed to be unclean and pest monitoring stations showed dead cock roaches in the FSO.

The PIC was able to provide an invoice documenting that a pest control contractor serviced the FSO on 8/30/19.
Some progress was made towards cleaning the the FSO, but additional cleaning needs to take place.

A follow-up inspection will be conducted on 9/12/19.
The FSO shall undergo extensive cleaning before 9/12/19.
Remove the unnecessary equipment and clutter from the basement storage room.
Additional enforcement action will take place if significant progress is not made with regards to cleaning and pest control.
