[?] A summary of the violations found during the inspection are listed below.

3717-1-04.1(C) / Food-contact surfaces - cleanability
Critical Corrected During Inspection Food contact surfaces not easily cleanable. Observed black bowls (NSF) scored and scratched and plastic peeling off on bowls at the front service line. PIC discarded bowl that was severly scratched.

3717-1-04.8(E)(3) / Single-service articles and single-use articles - storage.
Corrected During Inspection Single-service and single-use articles not protected from contamination. Observed toliet paper exposed above the prep table. PIC had employees wrap the items to protect them and will keep them in there original packaging or make sure there protected until used.

3717-1-04(I) / Nonfood-contact surfaces - materials.
Corrected During Inspection Nonfood-contact surfaces constructed of unapproved materials. Observed stickers wrapped on the handles of knives. PIC removed stickers from knives.

3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Observed prep sink not maintained clean, sink is being used to pre -rinse dishes.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Floors in the back prep/warewashing area not maintained clean. If floors are no longer smooth and easily cleanable they shall be replaced.

3701-21-03 / Facility layout and equipment specifications for FSO
Repeat FSO did not submit facility layout and equipment specifications. Observed lobby,restrooms, front service line, lighting, customer self-service area all appeared to be renovated as noted in previous inspection.

Observed renovations within the facility, plans were not submitted prior to construction. Submit plans to the health department as soon as possible.
Lighting may have been updated and shall be included in plans if adjusted. Unable to get the make and model of the front prep refrigeration unit. Customer self-service area appeared altered. Lobby and restrooms have been fully renovated.
The items listed above were noted on previous inspection. A plan review packet will be attached with the inspection report.

The violations documented shall be corrected and will be checked during next inspection.
