3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Corrected During Inspection Observed food (ready-to-eat foods stored under raw shell eggs) that was not properly protected from contamination by separation, packaging, and segregation at the time of inspection.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from ready-to-eat food as described in this rule. PIC moved the eggs to a separate shelf at the time of inspection.
3717-1-05.1(E) / Backflow prevention device - design standard.
Critical Repeat Observed no backflow prevention device installed for the hose connected to the water supply system for the service sink at the time of inspection
To prevent contamination, a backflow or backsiphonage prevention device installed on a water supply system shall meet American Society of Sanitary Engineering (ASSE) standards. Properly install a backflow prevention device.

3717-1-03.2(Q) / Food storage - preventing contamination from the premises.
Observed improper storage of food items (silver dollar rolls stored on the floor of the walk-in freezer) at the time of inspection.
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor.
3717-1-03.2(R) / Food storage - prohibited areas.
Observed food in the walk-in freezer was stored in an unapproved location (under dripping condensate) at the time of inspection.
To prevent contamination, food may not be stored under lines on which water has condensed.
3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Observed equipment component (reach-in freezer door) was not tight-fitting or properly adjusted at the time of inspection.
To ensure that the reach-in freezer continues to function properly, equipment components such as doors, seals, and hinges shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications. Repair/adjust the door to the reach-in freezer to be tight-fitting. Remove this equipment if it can not be repaired.
3717-1-06.4(A) / Repairing.
Repeat Observed damaged drywall behind the service sink at the time of inspection.
The physical facilities shall be maintained in good repair. Repair the drywall behind the service sink so that it is smooth, durable, and easily cleanable.
3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Repeat Observed an accumulation of dust on ventilation system above cooking surface at the time of inspection.
Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination.
