[?] A summary of the violations found during the inspection are listed below.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical TCS foods not being cold held at the proper temperature. Observed 3 bowls of butter above the prep cold holding unit at a temperature of 70 degrees F. PIC stated the product will be discarded and were from yesterdays event.

3717-1-03.4(H)(1) / Ready-to-eat, time/temperature controlled for safety food - disposition.
Critical Ready-to-eat, TCS foods not being date marked. Observed opened milk and half-n-half in the glass reach-in cold holding unit at the bar area without date marks. Observed containers of chicken, housemade sauce, cut lettuce, and half-n-half in the prep cold holding units at the main prep area without date marks. PIC stated they will contact the owner for proper correction.

Handwashing signs not posted at employee restrooms near the office.

3717-1-04.8(G)(3) / Kitchenware and tableware - furnishing single-service articles.
Single service articles intended for food or lip contact improperly dispensed. Observed beverage cups exposed and removed from its original packaging displayed next to the drink dispensing equipment near the front service counter.

3717-1-04.8(E)(3) / Single-service articles and single-use articles - storage.
Single-service and single-use articles not protected from contamination. Observed disposable containers removed from its original packaging stored on the shelving rack next to the warewashing machine in the main prep area.

3717-1-04.8(G)(1) / Kitchenware and tableware - handling of single-service and single use articles.
Single-service or single-use articles displayed or dispensed properly. Observed unwrapped large red straws displayed in multiple containers at the bar countertop.

3717-1-04.4(B) / Cutting surfaces.
Cutting blocks or boards cannot be effectively cleaned and sanitized. Observed discolored white cutting surfaces on the main prep cold holding unit.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Equipment components are not intact, tight or properly adjusted. Observed exterior of ice machine damaged and leaking water onto the floor. Observed damaged dessert display case. Observed damaged gaskets for both reach-in freezers and the 4-door reach-in cold holding unit.

3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Observed unapproved containers used for storing food (gladware, re-using sinlge-use containers) and equipment not approved by a recognized testing agency (microwave, toaster, blender, griddle, etc.).

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed an accumulation of food residue and other debris on non-food contact surfaces of equipment (popcorn machine, cookie display case, interior bar coolers, scale by fryers, drink dispensing equipment and nozzles, utensil holders, exterior of nacho chip holding equipment, etc.) throughout the facility.

3717-1-05.3(I) / Other liquid wastes and rainwater.
Repeat Improper disposal of condensation and other nonsewage liquids. Observed containers being used to collect water from the walk-in condensor unit, ice bin near the ice machine, and/or the 4-door cold holding unit, reach-in freezer, or main prep cold holding unit.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Facility not maintained clean. Observed floors under and around the cooking line, the ice machine, the bar area, dry storage areas, and all other hard-to-reach areas not maintained clean.
