Repeat Facility does not have an employee with manager certification in food protection from ODH.

3717-1-04.1(H) / Nonfood-contact surfaces - cleanability
Repeat Nonfood-contact surfaces not easily cleanable. Shelves in the deli display case and wooden shelves in the dry storage room are not easily cleanable.

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Unnecessary or nonfunctional items and /or litter on premises. There is litter near the dumpster. There is a large amount of old equipment on the premises (cold holding unit in the deli, sandwich conveyer in the deli, and unnecessary items stored in the rear portions of the building).

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Facility not maintained clean. There are multiple areas of the facility that are not clean (bathroom, dry storage room, and walk in cold holding units).

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. There are multiple areas of the facility in disrepair (papertowel dispenser, damaged ceiling tiles, and the floors, walls, ceilings, and floors junctures in and around the bathroom).

The following violation(s) have been corrected since the last inspection.
3717-1-02.4(C)(14)/PIC: duties - ensure employees are properly trained in food safety, including food allergy awareness.
Person in charge did not ensure their employees are properly trained in food safety. Multiple critical violations were caused by a lack of proper food safety amongst the staff (date marking, allergy information for labels, temperature violation, etc.). Education was provided to the PIC to improve food safety procedures going forward.
3717-1-06.2(C)/Handwashing sinks - hand drying provision.
No paper towels at the handwashing sink in the deli.
3717-1-03.4(F)(1)(b)/Time/temperature controlled for safety food - cold holding.
TCS foods not being cold held at the proper temperature. Multiple TCS foods in the cold holding display unit were holding at 50 degrees F (butter, milk, eggs, bacon, cheese). The PIC voluntarily discarded the food and a technician was hired to repair the equipment. TCS food will not be stored in the equipment until proper temperatures can be maintained.
3717-1-03.4(G)/Ready-to-eat, time/temperature controlled for safety food - date marking.
Refrigerated, ready-to-eat, TCS foods not properly date marked in the deli display case (turkey, chicken, roast beef, and sub sandwiches). Education was provided to the PIC to improve date marking procedures.
3717-1-03.5(C)(2)/Food labels - packaged food
Label on a food packaged in-house did not contain all required information. Items packaged in the deli (deli sandwiches and deli salads) do not contain all required information (sub-ingredients and address).
3717-1-03.5(C)(2)(e)/Food labels - label information
Missing and/or incomplete allergen declaration(s). Items packaged in the deli (deli salads and sub sandwiches) do not list the allergens properly on the label. Education was provided to the PIC about proper labels. The foods were removed from the open customer display case and placed behind the counter in the deli equipment until labels could be resolved.
3717-1-06.4(K)/Controlling pests.
Presence of live insects (drain flies) near the 3-compartment sink. Pest control options were discussed with the PIC.
3717-1-04.4(A)(2)/Equipment components kept intact, tight, and adjusted
Cold holding display equipment components are not properly adjusted to keep TCS foods at proper temperature.
