[?] A summary of the violations found during the inspection are listed below.

3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Repeat Observed a direct connection between the sewage system and a drain originating from equipment (warewashing sink used to wash fruits and vegetables) at the time of inspection.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment (warewashing sink) in which food, portable equipment, or utensils are placed. Install a culinary sink that has a proper air gap. Recommend contacting a licensed plumber and please contact the building department.

3717-1-02.4(A)(2) / Person in charge: assignment of responsibility.
Observed the facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection. The PIC has taken the course and has scheduled to take the test.
To meet the requirements of this rule, at least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code.
