3717-1-02.4(B) / Person in charge: demonstration of knowledge.
Critical The person in charge was unable to demonstrate proper knowledge of food safety and prevention at time of inspection.
Provide proof of PIC (person in charge) training level 1. Based on the risks inherent to the food operation, during inspections and upon request the person in charge shall demonstrate to the licensor the applicable food safety knowledge at the time of inspection.
3717-1-04.2(I) / Sanitizing solutions - testing devices.
Critical Observed no test kit available for measuring the concentration of the sanitizer at time of inspection.
A test kit or other device that accurately measures the concentration in PPM (mg/L) of sanitizing solutions shall be provided.
3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Equipment food-contact surfaces or utensils are dirty. Observed the cappucino machine dispenser nozzles to be dirty at time of inspection.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch.
3717-1-06.2(B) / Handwashing cleanser - availability.
Critical Observed no supply of hand cleaning liquid, powder or bar soap at the handwashing sink (near the ware washing station) at time of inspection.
Each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap.
3717-1-06.2(C) / Handwashing sinks - hand drying provision.
Critical Observed no towels or hand drying device at the handwashing sinks (employee restroom and near the ware washing station) at time of inspection.
Each handwashing sink or group of adjacent handwashing sinks shall provide hand drying by a means described in this rule.

3717-1-05.4(F)(1) / Outside receptacles - tight fitting lids
Observed outdoor refuse containers without tight fitting lids, doors, or covers a time of inspection.
Receptacles and waste handling units for refuse, recyclables, or returnables used outside with materials containing food residue, shall be designed and constructed to have tight-fitting lids, doors, or covers.
3717-1-06(A)(1) / Indoor areas - surface characteristics
Observed floors and walls that were not constructed of approved materials at time of inspection in the employee restroom.
Where FSO or RFE activities are conducted, materials for indoor floor, wall, and ceiling surfaces shall be smooth, durable, and easily cleanable; closely woven and easily cleanable carpet for carpeted areas; and nonabsorbent for areas subject to moisture.
3717-1-06.2(E) / Handwashing signage.
Observed no handwashing sign posted at handwashing sink near the ware washing station used by employees at time of inspection.
A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees.
3717-1-06.3(A) / Handwashing sinks - conveniently located.
Observed no conveniently located handwashing sink(s) at time of inspection near the ware washing station. The hand washing sink was blocked by a door and a ladder preventing employee access.
Handwashing sinks shall be conveniently located as specified under paragraph (L) of rule 3717-1-05.1 of this code.
3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Observed litter on the premises at time of inspection along the parking lot and near the dumpster area.
The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter.
