3717-1-04.5(A)(1) / Cleanliness of equipment food-contact surfaces and utensils.
Critical Observed equipment food-contact surfaces (can opener) that was dirty at the time of inspection.
To prevent contamination, equipment food-contact surfaces shall be clean to sight and touch. The can opener shall be cleaned and maintained to meet the requirements of this rule.

3717-1-02.2(G) / Jewelry - prohibition.
Observed food employees wearing jewelry (bracelets) on arms or hands at the time of inspection.
To prevent contamination, except as provided by (G)(2) and (G)(3) of this rule, food employees may not wear jewelry on their arms or hands. Employees are only permitted to wear a single wedding band, all other jewelry shall not be worn.

Overall the facility appeared very clean and well organized at the time of inspection. The person-in-charge was knowledgeable in food safety.
A level 2 ODH certification in food safety training is required to be obtained for one operator of the facility by March 1, 2017.
