[?] A summary of the violations found during the inspection are listed below.

3717-1-05.1(O)(2) / Using a handwashing sink - other uses prohibited.
Critical Observed ice in the handwashing sink behind the 1st floor bar at the time of inspection that indicated the handwashing sink was used for purposes other than handwashing at the time of inspection.
A handwashing sink may not be used for purposes other than handwashing.
3717-1-05.3(C) / Backflow prevention.
Critical Repeat Observed direct connections at the time of inspection between the sewage system and a drain originating from equipment (ice bins behind 1st and 3rd floor bars, and ice machine) in which food (ice) is placed.
A direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code. Eliminate these direct connections.

3717-1-03.2(M) / Wiping cloths - use limitation.
Observed improper maintenance of wiping cloths (wet wiping cloths in storage on 3rd floor bar) at the time of inspection.
Cloths that are in use for wiping food spills from tableware and carry-out containers shall be maintained dry and used for no other purpose; cloths in use for wiping counters and other equipment surfaces shall be held between uses in a chemical sanitizer, or laundered daily; cloths used with raw animal foods shall be kept separate from other cloths, dry cloths and solutions shall be free of food debris and visible soil; containers of sanitizer solutions shall be stored off the floor, and used in a manner that prevents contamination. Single-service articles, single-use articles or single-use disposable sanitizer wipes shall be used in accordance with EPA - approved manufacturer's label use instructions.
3717-1-06.2(E) / Handwashing signage.
Observed no handwashing signs posted at the time of inspection at handwashing sinks in men's and women's restrooms used by employees.
A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees.

Plumbing alterations should be made only by a licensed plumber.
Pre-existing non-commercial food equipment in the facility is approved for use as long as it continues to function properly and is maintained clean. All newly purchased equipment shall be commercial grade. Contact the City of Kent Health Department for approval before placing any new equipment into use.
