3717-1-02.3(A) / Food contamination prevention - eating, drinking, or using tobacco.
Critical Observed employee drinking in non-designated area with an unapproved beverage container at time of inspection. .
To ensure proper hygiene, an employee shall eat, drink, or use tobacco only in designated areas where the contamination of exposed food; clean equipment, utensils or linens; unwrapped single service or single use articles; or other items needing protection cannot result. Only approved beverage containers shall be used.
3717-1-05.1(S)(1) / Plumbing system - maintained in good repair.
Critical Observed the plumbing system of the 3 compartment ware washing sink was not in proper repair at time of inspection. Observed plumbing system was leaking at time of inspection.
A plumbing system shall be repaired according to the Ohio building code. Repair ware washing sinking plumbing to not be leaking.

3717-1-03.2(M) / Wiping cloths - use limitation.
Observed improper use and/or maintenance of wiping cloths at time of inspection.
Cloths that are in use for wiping food spills from tableware and carry-out containers shall be maintained dry and used for no other purpose; cloths in use for wiping counters and other equipment surfaces shall be held between uses in a chemical sanitizer, or laundered daily; cloths used with raw animal foods shall be kept separate from other cloths, dry cloths and solutions shall be free of food debris and visible soil; containers of sanitizer solutions shall be stored off the floor, and used in a manner that prevents contamination. Single-service articles, single-use articles or single-use disposable sanitizer wipes shall be used in accordance with EPA - approved manufacturer's label use instructions.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Observed cold holding equipment for pizza toppings was not in good working order. Pizza toppings cold holding equipment was not working at time of inspection. Ice was being used to maintain foods at acceptable temperatures.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. Repair the refrigerator to hold pizza at 41 degrees F or below without the use of Ice.
3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Observed accumulation of soil residue on nonfood-contact surfaces or the interior of pizza oven and conveyor at time of inspection.
Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
3717-1-04.8(A) / Equipment and utensils - air-drying required.
Observed beverage container are not being properly air dried before stacking at time of inspection.
After cleaning and sanitizing, equipment and utensils shall be air-dried or used after adequate draining before contact with food and utensils, and may not be cloth dried.
3717-1-06.4(D) / Cleaning ventilation systems, nuisance and discharge prohibition.
Observed the ventilation system over the pizza oven was not properly provided vent filters at time of inspection.
Provide filters on the ventilation system over the pizza oven. Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination. If vented to the outside, ventilation systems may not create a public health hazard or nuisance or unlawful discharge.
