[?] A summary of the violations found during the inspection are listed below.

3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Observed a direct connection between the sewage system and a drain originating from equipment in which food or utensils are placed. The ice machine and ice bin was observed with no air gap on the drainage lines at the time of inspection.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code for: a warewashing machine between its waste outlet and a floor drain when the machine is located within five feet (1.5 meters) of a trapped floor drain and the machine outlet is connected to the inlet side of a properly vented floor drain trap; or a warewashing and culinary sink. An air gap shall be installed on the drainage lines for the ice machine and ice bin.

3717-1-04.8(E) / Equipment, utensils, linens, single-service articles, and single-use articles - storage.
Observed boxes of single-service articles or single-use articles were stored on the floor in the back storage room at the time of inspection.
To prevent health hazards, single-service articles, and single-use articles shall be properly store: In a clean, dry location; where they are not exposed to splash, dust, or other contamination; and at least six inches (fifteen centimeters) above the floor. Discontinue storing boxes of single use items on the ground.
3717-1-05.1(B)(1) / Approved plumbing system.
Repeat Observed the plumbing system on the 3 compartment sink does not comply with the Ohio building code. Observed the drainage on the 3 compartment sink to be leaking and observed a strong sewer odor at the time of inspection.
To prevent health hazards, a plumbing system shall be designed, constructed, and installed according to the Ohio building code. Have the plumbing system on the 3 compartment sink and grease interceptor inspected by a licensed plumber and make repairs/corrections where needed.
3717-1-05.1(Q) / Scheduling inspection and service for a water system device.
Observed a water treatment device for the ice machine not inspected or serviced as required at the time of inspection.
To prevent health hazards, a device such as a water treatment device or backflow preventer shall be inspected and serviced in accordance with manufacturer's instructions and as necessary to prevent device failure, and records demonstrating inspection and service shall be maintained by the person in charge. Water filter for the ice machine shall be serviced.

Comments
The FSO was currently only serving drinks and snacks for the summer hours.
All temperatures measured were acceptable at the time of inspection.

Shelving that employees are permitted to store personal belongings/foods shall be labeled and maintained separate from food items for customers. A container labeled (For Employee use only) may help maintain this separation. A lot of the food is unlabeled and not dated. It is unclear what is for the customers and what is employee foods.

The person in charge shall ensure that food and conditional employees are informed in a verifiable manner of their responsibility to report information about their health as it relates to diseases that are transmissible through food. Please refer to the handout provided.

Due to the lack a food processing sink (sink with a indirect connect to the sewer and air gapped drain) at this location, a restriction is required for this FSO as follows: Operation has not been provided with a properly installed Food Processing sink. All food shall be properly washed, prepared, and provided with a date of preparation from supplier before being delivered to this location.

An employee with managerial duties shall have a level 2 ODH certification in food protection. The level 2 certification shall be available upon request.

The violations documented on this report will be checked during next standard inspection.
