3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Repeat Observed equipment (reach-in cooler) was not maintained in good working order at the time of inspection.
To ensure that refrigerated time/temperature-controlled for safety (TCS) food is maintained at an acceptable temperature that is 41°F or less, this equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. This refrigerated equipment may not be used for long-term storage of TCS foods. Any TCS food stored in this unit shall be discarded after 4 hours

The person-in-charge stated that the reach-in refrigeration unit was serviced, and that a new reach-in refrigeration unit was ordered to replace the failing equipment. The PIC showed documentation that a new reach-in refrigeration unit was purchased. This new equipment is expected to be delivered in the next 5 to 7 days.
