[?] A summary of the violations found during the inspection are listed below.

3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Observed a direct connection between the sewage system and a drain originating from equipment in which food are placed (customer self-service ice bin) at the time of inspection. The drainage lines from the ice bin had an air break from the floor drain.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment in which food. The drainage line shall contain a proper air gap.

3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Observed accumulation of soil residue on nonfood-contact surfaces (nozzle of suppression system for whopper station and ice scoop holder by the drive thru) at the time of inspection.
To prevent contamination, nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. The equipment listed above shall be cleaned and maintained.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of food debris or grease on the floors in the following areas at the time of inspection: 1) near the BOSS grease receptacle 2) near the drive thru window (under shelving) 3. Inside the slushie equipment.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.

Working containers holding food or food ingredients that are removed from their original packages shall be identified with the common name of the food, except containers holding food that are readily and unmistakably recognized.
Equipment and utensils must be air dried to become properly sanitized. Recommend using table mesh for the drainboard to help with air drying or installing a drying rack.
A level 2 ODH certification in food safety training for one operator of the facility shall be verifiable.
Violations documented on this report will be checked during next standard inspection.
