Employees are not informed in a verifiable manner of their responsibility to report information about their health. Signed employee sick policy not available for all employees. Education was provided about proper policy.

Food employee did not wash hands when required; after contaminating hands and before donning gloves to prepare food. Education was provided to improve hand washing procedures.
