[?] A summary of the violations found during the inspection are listed below.

3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Equipment is not approved by a recognized testing agency. Observed multiple non-commercial coolers and a freezer in use at time of inspection.
To prevent health hazards, discontinue the use of non-commercial equipment. Food equipment that is acceptable for use in a FSO or RFE shall be approved by a recognized food equipment testing agency.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Repeat Equipment and/or components are not maintained in good working order. Observed two non-working commercial coolers in the kitchen at time of inspection.
To promote proper cold holding, repair the commercial coolers. Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.

3717-1-03.2(C)/Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Observed food that was not properly protected from contamination by separation, packaging, and segregation. Raw eggs were stored over lettuce at time of inspection.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule.
3717-1-03.4(G)/Ready-to-eat, time/temperature controlled for safety food - date marking.
Observed refrigerated, ready-to-eat, TCS foods held refrigerated for more than 24 hours were not properly date marked at time of inspection (lettuce, rice, tomatoes, grape leaves, hummus, watermelon, etc.).
To prevent foodborne illness, refrigerated, ready-to-eat, TCS food held at a temperature of 41°F or less for more than 24 hours shall be clearly marked at the time of preparation or the time the original container is opened to indicate the date or day, that is a maximum of seven days, by which the food shall be consumed, sold, or discarded. The day the food is prepared or opened is day one.
3717-1-04.1(Y)/Temperature measuring devices.
Cold or hot holding equipment did not contain a thermometer, or the thermometer was not correctly placed. Observed multiple coolers without thermometers at time of inspection.
To ensure proper temperatures, provide thermometers in all coolers. A properly designed temperature measuring device as specified in this rule shall be located in the warmest part of a cooling unit or the coolest part of a food warming unit.
3717-1-04.5(A)(1)/Cleanliness of equipment food-contact surfaces and utensils.
Equipment food-contact surfaces or utensils are dirty. Observed the can opener blade to be dirty at time of inspection.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch.
3717-1-04.5(A)(3)/Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are dirty. Observed multiple pieces of equipment to be dirty at time of inspection (preparation tables, microwave, coolers, shelves, etc.).
To prevent contamination, clean the equipment. Nonfood-contact surfaces of equipment shall be kept clean.
3717-1-06.4(B)/Cleaning - frequency and restrictions.
Observed a build-up of dirt, dust, grease, and debris at time of inspection in the following areas: walls of the kitchen, exhaust hood, and floors of the kitchen and preparation areas.
To prevent contamination, clean the FSO. The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.
Comments
Please ensure that the non-working coolers have been repaired/replaced ASAP.
Please contact the Health Department prior to purchasing future equipment (coolers, etc.). There are several unapproved coolers and a freezer in the kitchen.
The new "Carrier" cooler with the sliding glass doors is only permitted to be used for packaged products.
