| This inspection was conducted in response to a request for a new Retail Food Establishment license and to complete a facility review. The operator has completed all items required and has been approved to be licensed. A signed application and payment for the amount of $213.00 (Check#1129) for license #JSMH-9VNG76 was received at the time of inspection. Proof of Person In Charge training must be provided. Note: Provide self closing and self latching doors to the back entrance/exist of the warehouse. Provide a Certificate of Occupancy once it has been provided by the Building/fire department. Okay to operate. License will be available for pick-up at the Kent Health Department once processing has been completed. |
