Handwashing signs not posted at the main handwashing sink and in the restrooms.

3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Observed knife holders at the front service line not maintained clean. Observed prep sink not maintained clean, sink is being used to pre -rinse dishes.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed damaged wall materials on corners and bottom section of walk-in and covebase.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Floors in the back prep/warewashing area not maintained clean. If floors are no longer smooth and easily cleanable they shall be replaced.

3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
Floor and wall junctures not properly coved or closed and/or floor drains not provided. Observed missing baseboard in the men's restrooms.

3701-21-03 / Facility layout and equipment specifications for FSO
FSO did not submit facility layout and equipment specifications. Observed lobby,restrooms, front service line, lighting, customer self-service area all appeared to be renovated.

Observed renovations within the facility, plans were not submitted prior to construction. Submit plans to the health department as soon as possible.
Lighting may have been updated and shall be included in plans if adjusted. Unable to get the make and model of the front prep refrigeration unit. Customer self-service area appeared altered. Lobby and restrooms have been fully renovated.
Single use items such as toilet paper shall not be removed from the original packaging until used or if stored in a container.
The violations documented shall be corrected and will be checked during next inspection.
