[?] A summary of the violations found during the inspection are listed below.

3717-1-03.4(G) / Ready-to-eat, time/temperature controlled for safety food - date marking.
Critical Observed refrigerated, ready-to-eat, TCS foods (lettuce, spinach, cabbage) held refrigerated for more than 24 hours that were not properly date marked at the time of inspection.
To prevent foodborne illness, refrigerated, ready-to-eat, TCS food held at a temperature of 41°F or less for more than 24 hours shall be clearly marked at the time of preparation or the time the original container is opened to indicate the date or day, that is a maximum of seven days, by which the food shall be consumed, sold, or discarded. The day the food is prepared or opened is day one. Food items that are found without dates shall be discarded.

3717-1-04.4(Q) / Utensils, temperature and pressure measuring devices - good repair and calibration.
Observed the final rinse thermometer gauge for the warewashing machine not kept in good repair or calibration at the time of inspection.
To ensure equipment is functioning properly, water temperature measuring devices shall be maintained in good repair and be accurate within the intended range of use. Calibrate final rinse temperature gauge to meet the requirements of this rule.
3717-1-06.1(B) / Floors, walls and ceilings - utility lines.
Repeat Observed drainage pipes from the ice carts were improperly installed to be on the floor and draining water on to the floors at the time of inspection. Observed drainage pipes from the walk-in condenser units to be draining on to the floor tiles at the time of inspection.
To prevent health hazards, utility service lines and pipes may not be unnecessarily exposed, may not be installed on the floor, and shall be installed so they do not obstruct or prevent cleaning of the floors, walls, or ceilings. Drainage pipes shall be draining directly into the floor drains to prevent damage to the flooring. Constant water on the floors from equipment drainage pipes provides a source of water for pests. Install the drainage lines to drain directly into floor drains with proper air gaps.
3717-1-06.4(A) / Repairing.
Observed the physical facilities were not being maintained in good repair at the time of inspection. The caulking to the left of the hand washing sink located by the front service line was damaged.
To prevent health hazards, the physical facilities shall be maintained in good repair. Recaulk the counter and roll-in cold holding unit juncture to allow for easy cleaning and to maintain the facilities in good repair. Complete by Next Standard inspection.

Comments
Overall the facility was observed to be in good order. Manager was very knowledgeable in food safety.
The walk-in cold holding condenser shall be clear of dust and floors shall be smooth, easily cleanable and kept clean.
Please provide the policy and procedures for the shared food cart and foods held on ice.
A device such as a water treatment device/filter shall be scheduled for inspection and service, in accordance with manufacturer's instructions and as necessary to prevent device failure based on local water conditions, and records demonstrating inspection and service shall be maintained by the person in charge.
An email will be attached to this report regarding packaged foods that are monitored by food employees.
All violations documented on this report will checked for proper corrective action during the next standard inspection.
