[?] A summary of the violations found during the inspection are listed below.

3717-1-05.1(O)(2) / Using a handwashing sink - other uses prohibited.
Critical Handwashing sink is being used for purposes other than handwashing. Observed drain lines from the drink dispensing equipment at the front counter draining into the hand sink. PIC stated they pulled the drain lines from the floor drain to clean the drain last night and will fix it today.

3717-1-06.1(M) / Outer openings - protected.
Repeat Outer opening not protected. Observed no self-closing and self-latching back door used for receiving foods and taking out trash.

3717-1-03.2(G) / Washing fruits and vegetables.
Raw fruit and/or vegetables not washed or washed improperly. Per discussion with PIC, produce is being washed in a colinder at the three-compartment sink. Observed food prep sink blocked and inaccessible.

3717-1-04.4(B) / Cutting surfaces.
Multiple green cutting trays cannot be effectively cleaned and sanitized. Observed discoloring and scoring.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Observed an accumulation of food residue or other debris on the walk-in shelving units, oil mixture containers under finish table, and countertop area around front handwashing sink.

3717-1-06.1(A) / Floors, walls, and ceilings.
Ceilings not smooth and easily cleanable in the walk-in units.

3717-1-06.4(A) / Repairing.
Physical facilities not maintained in good repair. Observed ceiling tiles above the front service counter being held up by cardboard and paint stirs. Observed damaged ceiling in this area. Observed damaged walls for the walk-thru from the prep to cook area.

3717-1-06.2(I)(3) / Lighting - intensity (50 FC)
Light intensity less than fifty foot candles in required areas. Observed prep table moved to under the old exhaust hood in the prep room and no light bulbs were in the fixtures. Provide shatter proof light bulbs of install protective shields on a standard light bulb. Ensure lighting is at 50 foot candles.

A food thermometer shall be readily available at all times. Ensure thermometers are being tested for accuracy and calibrated if needed.
Continue to repair walls and surfaces that are no longer smooth and easily cleanable.
Handwashing sinks shall be properly supplied at all times. Observed jammed paper towel dispenser at the hand sink near the three-compartment sink.
Please provide the model number for the Delfield sandwich prep table.
Flooring that is no longer smooth and easily cleanable shall be replaced.
Ensure HVAC filters are being changed to help reduce dust building up around the ceiling vents.
The prep room has been rearranged and prep tables and other equipment have been removed.

The violations documented shall be corrected and will be checked during next standard inspection.
