In order Health Department approval to re-open the Food Service Operation (FSO) the following must be completed: 1) Caulk along all wall and counter top junctures around the Customer Drink bar and beverage area by drive-thru window and around the new food preparation sink 2) Separate the drains from the over flow drain and the Ice bin at the Drink Bar 3) Provide documentation that a licensed Pest Control Operator was hired to help with the flying insect issue 4) Cornellis CB3023-AV and ED300-BCZ are both new beverage dispensing equipment installed 5) Seal all holes in the FRP wall material throughout the facility 6) Provide Soap, a means to dry hands, Employees Must Wash Hands signage and trash can with lid (Women's Restroom) at all hand washing sinks 7) Ensure that Ventilation (exhaust) is installed in both rest-rooms 8) Seal the hole in the wall between the walk-in freezer and cooler where the condensate drain has been installed. Ice accumulation has already occurred in the freezer. 9) Provide a Air Gap (1 to 2 inches) on the condensation drain from the walk-in units 10) New hot water tank has been installed with 75,600 BTU's with a 72.82 gall recovery which meets the minimal spec according to the FDA requirements 11) Re-caulk around the mop sink 12) Replace the Mop Sink Faucet fixture or provide backflow prevention device on hose and water source 13) Separate the drainage from the ice maker and the ice bin - both are required to be air gapped with an indirect connection 14) Provide backflow prevention devices on all equipment water lines (Ice machine, ice machine, beverage dispensing equipment if not already provided, etc.) 15) Provide documentation from final building and fire departments approving the occupancy of the facility 16) All cold holding equipment shall be functioning properly and supplied with the proper temperature measuring devices in the correct locations 17) Provide access to the location of the grease interceptor or have knowledge of where the location is and the last time if was serviced This list is contingent upon additional review of the facility.
