The FSO is required to receive food from an approved vendor. Foods from other operations may need to be registered with the Ohio Department of Agriculture (ODA) as a processor. Documentation shall be obtained.
Papa John's and Chick-fil-a were mentioned as possible providers.
Ensure water filters are being maintained according to the manufacturer.

The facility was not operating during the inspection.
The violations documented shall be corrected and will be checked during next inspection
