[?] A summary of the violations found during the inspection are listed below.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Repeat Corrected During Inspection TCS foods not being cold held at the proper temperature. Observed shamrock farm milks held in the coca-cola cold holding unit above 41 degrees F (49). PIC voluntarily removed product from sale and will be discarded.

3717-1-05.1(J) / Backflow prevention device - when required.
Critical Repeat No approved backflow prevention device on the plumbing system for the ice tea dispensing equipment at the serving area. Observed shut-off type valve on the water supply line. Please provide documentation on the device installed if it is rated for backflow prevention (ASSE 1022 etc)

3717-1-03.2(K) / In-use utensils - between-use storage.
Repeat In-use utensils improperly stored. Observed utensils on the prep line used for the meatballs, chicken, and guac being placed on soiled wax paper. Utensils shall be stored on a surface that can be cleaned and sanitized at the required frequency or stored in the poroduct.

3717-1-04.8(G)(1) / Kitchenware and tableware - handling of single-service and single use articles.
Single-service or single-use article not handled, displayed, or dispensed properly. Observed beverage containers displayed exposed on the front service counter.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Floors in the facility specifically around the grease trap not maintained clean. Damaged floor tile in the dish area shall be repaired if flooring is no longer easily cleanable.

3717-1-06.2(I)(1) / Lighting - intensity (10 FC)
Repeat Light intensity less than ten foot candles in required areas. Observed measured light intensity of 4 foot candles in the walk-in cold holding unit.

3717-1-02.4(B)(2)(k)/PIC: Demonstration of Knowledge - Explaining correct procedures for cleaning and sanitizing
PIC unable to demonstrate knowledge of cleaning and sanitizing of utensils. PIC stated the utensils stored on wax paper on the prep line are cleaned and sanitized daily. PIC was informed utensils that are used for TCS foods like meatballs, chicken, guac are required to be warewashed at minimum every 4 hours.
3717-1-04.4(F)(2)/Warewashing sinks - cleaning and sanitizing.
Food preperation sink (also used to pre-soak dirty utensils and equipment) not properly cleaned and sanitized. Observed colinder equipment in the sink and the bottom and walls of the sink with an accumulation food debris.

The remaining violations shall be corrected and will be checked during the follow-up inspection.
