| This inspection was completed in response to a request from the new operator for the Licensee name to be changed. Based on previous inspection report violations and a recent Facility Review that was completed in 2013. No changes were observed in the facility based on the 2013 review. License is okay to be transferred into Bob Evans Restaurants, LLC from Bob Evans Farms, LLC. No Facility Review is required for this transfer. |
