[?] A summary of the violations found during the inspection are listed below.

3717-1-05.3(C) / Backflow prevention.
Critical Repeat Observed a direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. Observed ready to eat foods were being washing in the 3 compartment ware washing sink at time of inspection.
A direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code. Provide a food preparation sink that is properly installed with a in directly connected drainage to prevent contamination to foods. Until this sink is provided all foods that need to be washed shall be washed at the Stanton Middle school kitchen. Restrictions may be put on FSO license to restrict the preparation of unwashed foods.

3717-1-04.1(KK) / Food equipment - certification and classification.
Observed Pasta Salad was delivered and stored in a container (sterilite storage container) that was not approved by a recognized testing agency at the time of inspection.
Food equipment that is acceptable for use in a FSO or RFE shall be approved by a recognized food equipment testing agency. To prevent contamination to foods, only approved equipment shall be used. Food shall only contact surfaces that are food grade. Discontinue using the Sterilite containers in direct contact with ready to eat foods.
3717-1-06.1(M) / Outer openings - protected.
Observed an outer opening door without proper protection against the entry of insects and rodents. Observed back door to the kitchen was not self-closing at the time of inspection.
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule. Provide the back door to kitchen to be self-closing, self latching and tight-fitting.

Person In Charge mentioned that there are days when there is not enough cold holding for all the foods that require cold holding. If this continues then additional cold holding will be required at this location. Ice is not an approved method to cold hold TCS foods.
Observed a slight odor coming from the grease trap at the time of inspection. Recommend having the grease trap serviced.
Make sure all employees are aware of the when hands are to be washed.
Person In Charge demonstrated excellent knowledge in food safety.
Observed all monitoring employees were washing hands upon arrival to work.
