3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Corrected During Inspection Observed food (salad mix stored under raw shell eggs) that was not properly protected from contamination by separation, packaging, and segregation at the time of inspection.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from ready-to-eat food. PIC rearranged the foods in the 4 door reach in cooler so that RTE was protected from cross contamination.
3717-1-04.5(B) / Equipment food-contact surfaces and utensils - cleaning frequency.
Critical Observed food equipment surfaces of spatulas and the fry cutter were not cleaned at the required frequency at the time of inspection.
To reduce the risk of foodborne illness, equipment food-contact surfaces and utensils if used with time/temperature controlled for safety food, shall be cleaned throughout the day at least every four hours.
3717-1-05.3(C) / Backflow prevention.
Critical Repeat Observed no culinary sink with an air gapped drain line at the time of inspection for the washing of vegetables. Operation washes vegetables in the far left basin of the 3 compartment sink.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. Provide a culinary sink with air gapped drainage for the washing of vegetables.

3717-1-04.1(KK) / Food equipment - certification and classification.
Observed equipment (microwaves, crock pots) at the time of inspection that was not approved by a recognized testing agency.
Food equipment that is acceptable for use in a FSO shall be approved by a recognized food equipment testing agency. Remove this unapproved equipment.
3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Observed equipment component (torn door seal for reach in freezer) was not intact at the time of inspection.
To ensure that this equipment continues to function properly, equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of dirt and debris on the floors behind the bar at the time of inspection.
To limit sources of contamination, the physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.

A thermometer was properly located for monitoring the temperature of the 4 door cooler.
A sign reminding employees to wash hands was posted at the handwashing sink in the men's restroom.
Dunnage racks appeared clean and free of dust build-up. Please continue to maintain all equipment clean to sight and touch, ensuring all utensils such as tongs and other food equipment are protected from contamination during storage.
Food is received from GFS, Sysco, and Sam's Club. Continue to monitor supplied food packages to ensure they are received intact.
