[?] A summary of the violations found during the inspection are listed below.

3717-1-03.4(F)(1)(b) / Time/temperature controlled for safety food - cold holding.
Critical Corrected During Inspection TCS foods not being cold held at the proper temperature. Observed opened bag of cheese held at room temperature in the gehl warming equiment. Equipment was unplugged, the warming equipment shall be on and holding the product above 135 degrees F at all times. PIC discarded the cheese.

3717-1-03.4(H)(1) / Ready-to-eat, time/temperature controlled for safety food - disposition.
Critical Ready-to-eat, TCS food not properly discarded when required. Observed containers of lettuce, cut tomatoes, chicken, coleslaw, hot dogs, etc. without date marks.

3717-1-06.4(K) / Controlling pests.
Critical Flying insects throughout the area behind the bar (at hand sink, trash cans, cleaning bucket). Observed mouse pesticide packets in corners throughout the building. PIC stated they are working with a licensed pest control company.

3717-1-05.1(C)(1) / Handwashing sink - required water temperature
Handwashing sink water below 100°F at all hand sinks in the facility. PIC turned the temperature setting up on the water heater and will ensure the water reaches 100°F.

3717-1-03.2(K) / In-use utensils - between-use storage.
In-use utensils improperly stored. Observed ice scoops stored in the ice bin with handle not up out of the food. Observed employee scooping ice with glass cup.

3717-1-04.8(E)(2) / Clean equipment and utensils stored in a self-draining position and covered or inverted.
Clean equipment and utensils (plates, bowls, etc.) not stored in a self-draining position and covered or inverted in the main prep area.

3717-1-04.8(E)(1) / Single-service and single-use articles - storage
Observed boxes throughout the facility stored on the floor. Boxes shall be stored off the floor for protection and for floors to be easily cleanable.

3717-1-04.4(A)(2) / Equipment components kept intact, tight, and adjusted
Repeat Equipment components are not intact, tight or properly adjusted. Damaged gaskets for refirgeration and freezer units throughout.

3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Equipment not approved by a recognized testing agency. Observed unapproved containers used for storing food and equipment (microwave,toaster,blender,griddle,etc.).

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Observed an accumulation of food residue and debris on the door handles, doors, gaskets, interior of coolers, equipment at the grill line, potato chip equipment, and red bar glass mats.

3717-1-05.1(S) / Plumbing system - maintained in good repair.
Plumbing system not properly maintained or repaired. Observed damaged plumbing fixture for the bar hand sink near the warewashing machine.

3717-1-05.3(I) / Other liquid wastes and rainwater.
Improper disposal of condensation and other nonsewage liquids. Observed condensation drain lines from the ice bin, walk-in, and preprefigeration unit draining into containers.

3717-1-06.4(H) / Cleaning and maintenance of plumbing fixtures.
Plumbing fixtures unclean. Observed debris building up on plumbing fixtures throughout the facility (handsinks, spray nozzle, three compartment sink,etc.)

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Floors throughout the facility not maintained clean.

3717-1-06.1(A) / Floors, walls, and ceilings.
Floors not smooth and easily cleanable throughout the facility, specifically under and around equipment.

Please provide the make and model number of the pizza tabletop oven. Please consult with the fire department for the use of this equipment in the current location.
Original packaging shall not be removed until cups are being used by customers.
Ensure all spray bottles for cleaning and sanitizing are being labeled with the common name.
The prep sink shall be cleaned and sanitized before and after use.

The violations documented shall be corrected and will be checked during the follow-up inspection was scheduled for Friday, April 29.
