3717-1-02.4(A)(2) / Person in charge: assignment of responsibility.
Repeat Observed facility did not have one employee with supervisory and management responsibility and the authority to direct and control food preparation and service with level two certification in food protection at time of inspection.
To prevent health hazards, at least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall obtain the level two certification in food protection according to rule 3701-21-25 of the Administrative Code.
3717-1-06.1(C) / Floor and wall junctures - coved, and enclosed or sealed.
Repeat Observed the floor and wall junctures are not properly coved or closed at time of inspection.
To promote proper cleaning, provide sealed or coved wall junctures. In FSO's or RFE's in which cleaning methods other than water flushing are used for cleaning floors, the floor and wall junctures shall be coved and closed to no larger than one thirty-second inch; where water flush cleaning methods are used, shall be provided with drains and be graded to drain, and the floor and wall junctures shall be coved and sealed.
3717-1-06.4(A) / Repairing.
Repeat The physical facilities are not being maintained in good repair. Observed the walk in freezer condenser in disrepair. Observed multiple areas of the back room in disrepair (ceiling tiles, walls, and floor) at time of inspection.
To prevent health hazards, repair the facility. The physical facilities shall be maintained in good repair.

Observed the FSO did not have a person in charge that had completed a Level One Certification course at time of inspection.
To prevent health hazards, provide training to all deli staff.
3717-1-02.4(B)/Person in charge: demonstration of knowledge.
Observed the person in charge was unable to demonstrate proper knowledge of food safety and prevention at time of inspection (date marking procedure).
To prevent or correct factors that may cause foodborne illness, based on the risks inherent to the food operation, during inspections and upon request, the person in charge shall demonstrate to the licensor the applicable food safety knowledge at the time of inspection.
3717-1-02.4(C)(14)/Person in charge: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. Observed the RFE without signed employee reporting agreements at time of inspection.
To prevent transmission of foodborne illness, the person in charge shall ensure that food and conditional employees are informed in a verifiable manner of their responsibility to report information about their health as it relates to diseases that are transmissible through food.
3717-1-03.4(G)/Ready-to-eat, time/temperature controlled for safety food - date marking.
Observed refrigerated, ready-to-eat, TCS foods held refrigerated for more than 24 hours were not properly date marked at time of inspection (chicken, ham, turkey, roast beef, corned beef, and sub sandwiches).
To prevent foodborne illness, refrigerated, ready-to-eat, TCS food held at a temperature of 41°F or less for more than 24 hours shall be clearly marked at the time of preparation or the time the original container is opened to indicate the date or day, that is a maximum of seven days, by which the food shall be consumed, sold, or discarded. The day the food is prepared or opened is day one.
3717-1-03.5(C)(2)/Food labels - packaged food
A label on a food packaged in the RFE or FSO did not contain all required information. Observed packaged sub sandwiches without sub-ingredients listed on the label at time of inspection.
To prevent health hazards, provide proper labels. Foods shall be labeled as specified in this rule.
3717-1-06.2(E)/Handwashing signage.
Observed no handwashing sign posted at handwashing sink used by employees in the restroom at time of inspection.
To promote proper handwashing, a sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees.
