3717-1-02.4(C)(14) / Person in charge: duties - ensure employees are informed in a verifiable manner of their responsibility to report their health information
Critical Person in charge did not ensure that employees are informed in a verifiable manner of their responsibility to report information about their health. Observed the RFE without signed employee reporting agreements at time of inspection.
To prevent transmission of foodborne illness, the person in charge shall ensure that food and conditional employees are informed in a verifiable manner of their responsibility to report information about their health as it relates to diseases that are transmissible through food.

3717-1-05.4(B) / Outdoor storage surface.
Repeat Observed the outdoor refuse storage surface is not properly constructed, and sloped to drain at time of inspection.
To prevent health hazards, provide the dumpster with a slab (concrete or asphalt). An outdoor storage surface for refuse, recyclables, or returnables shall be constructed of nonabsorbent material and shall be smooth, durable, and sloped to drain.
3717-1-06.1(M) / Outer openings - protected.
Observed an outer opening, window, or door without proper protection against the entry of insects and rodents. The back door was not self-latching at time of inspection.
Allow the back door to self-latch. To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule.

Obtain the Level 2 certification in food protection from the Ohio Department of Health.
Provide a final Building and Fire Department approval and/or certificate of occupancy.
Provide a proper air gap on the drainage lines from the hot water tank and AC unit.
Provide kitchen grade ceiling tiles in the food preparation room.
Caulk along the doorway junctures.
The PIC stated that an air drying rack is scheduled to be installed in the near future above the 3-compartment sink. Wooden shelves should be avoided in the RFE.
"Kitchen Rayyan" provides packaged food (hummus, grape leaves, and tabouli) to Kent Natural Foods Co-op. Confirm that they are an approved source and follow proper labeling requirements and food safety procedures.
