Observed equipment (reach-in cooler) was not maintained in good working order at the time of inspection.
To ensure that refrigerated time/temperature-controlled for safety (TCS) food is maintained at an acceptable temperature that is 41°F or less, this equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. This refrigerated equipment may not be used for long-term storage of TCS foods. Any TCS food stored in this unit shall be discarded after 4 hours

All violations were corrected. The new cooler is holding at 38 degrees F. The new cooler has been approved for commercial use.
