3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Corrected During Inspection Food not properly protected from contamination by separation, packaging, and segregation. Raw chicken stored over RTE foods in preparation cold holding unit. The PIC immediately moved the items once the violation was identified.

3717-1-06.1(K) / Insect control devices - design and installation.
Corrected During Inspection Improper placement of insect control devices at the patio bar placed over the 3-compartment sink. The PIC was able to remove the item once the violation was identified.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Non-food contact surfaces of equipment are unclean. Multiple pieces of equipment are not clean in the kitchen (cold holding units, cooking equipment, sinks, exhaust hoods, storage racks and shelving).

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. Multiple damaged floor tiles and ceiling tiles throughout the building.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Floors are not maintained clean throughout the building (bar area, basement, mop room, bathroom, and kitchen).

3717-1-06.4(N) / Maintaining premises - unnecessary items and litter.
Repeat Unnecessary or nonfunctional items stored in the basement (old equipment).

Staff is very knowledgeable about proper food safety. Improve cleaning.
The FSO is currently licensed as a risk class 3, but performing risk class 4 activities (cooling and reheating in bulk). The PIC is knowledgeable about proper procedures, but next licensing period will require an increase in risk class.
