[?] A summary of the violations found during the inspection are listed below.

3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Repeat Observed a direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed. Observed the facility was not provided with a designated, properly installed sink for preparing ready to eat foods at the time of inspection.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code. Provide a food preparation sink that is properly installed with a in directly connected drainage to prevent contamination to foods. Until this sink is provided, all foods that need to be washed shall be washed at the Stanton Middle school kitchen.
3717-1-07.1(C) / Poisonous or toxic materials: Conditions of use.
Critical Observed unapproved application of sanitzer (very high concentration) in the buckets used for the cafeteria tables at the time of inspection.
To prevent health hazards, toxic materials shall be used and applied according to: (a) Law and this chapter; (b) Manufacturer's use directions included in labeling, and, for a pesticide, manufacturer's label instructions that state that use is allowed in a food service operation, retail food establishment, or food handling or processing areas. Monitor the concentration of sanitizer buckets and ensure the label is followed.

3717-1-06.1(M) / Outer openings - protected.
Repeat Observed an outer opening door without proper protection against the entry of insects and rodents. Observed back door to the kitchen was not self-closing at the time of inspection.
To prevent contamination, outer openings of an FSO shall be protected against the entry of insects and rodents as specified in this rule. Provide the back door to kitchen to be self-closing, self latching and tight-fitting.

Handwashing shall occur once arrived at work and before starting the first task.
Ensure labels are retained and maintained at the facility for products that are removed from boxes (chicken patties, corn dogs, etc.)
The violations documented shall be corrected as soon as possible and will be checked during next standard inspection.
