Comments
This consultation inspection was completed as part of the pre-licensing process and per the request of the Manager/owner. The following list shall be completed before a FSO risk Level IV license will be issued for the facility:

1) Seal the back bar countertop to the brick wall. Juncture shall be coved.
2) Provide space between sinks and equipment around the bar for cleaning or push the sinks and equipment together and seal spaces with caulk.
3) Seal all sinks and no movable equipment to the under bar wall. 3 compartment sink shall be install with no space between the wall and the back space.
4) All light over the bar shall be shatter proof. Provide shatter proof bulbs without the glass globe or shatterproof globes or fixtures.
5) Provide a trash can with a lid in the women's restroom.
6) Ensure all hand washing sinks are able to be provided with at least 100 degrees F water.
7) Ensure all hand washing sinks are provided with proper employees must wash hands signage, soap and paper towels.
8) Caulk around the kitchen 3 compartment sink grease interceptor and floor juncture. Provide a coved finish.
9) Provide a coved base finish around all wall and floor junctures. Provide additional grout or caulking.
10) Seal all steel backspace junctures behind the hot line.
11) Dish machine shall be installed, supplied with proper chemicals and calibrated.
12) Provide a proper test kit for the sanitizer used for the dish machine.
13) Provide an additional Quat test kit for the front bar.
14) Provide an alarming device on the dish machine that provides a visual or audible alarm when detergent and sanitizer is empty.
15) Install FRP or similar material behind the soda dispensing equipment in basement.
16) Provide a "bleed" drainage line on the backflow prevention device on soda dispensing compressor.
17) Finish installing Ice machine. Ensure drain from maker and bin are separated and provided with proper air gaps. Ensure a backflow prevention device is installed on the water connection.
18) Beer cooler floor shall be sealed, replace missing wood, and provide proper coved base finish along all floor and wall junctures.
19) Provide at least 10 foot candles of lighting in beer cooler. Additional light maybe needed.
20) Beer cooler and beer line cooler shall be draining to a sanitary sewer drain and provided with a proper air gap.
21) Basement restroom is require to have a working window or exhaust fan the vents to the outside.
22) Stairs to basement shall sealed and finished with a coved bases (caulk and seal all junctures and paint or seal exposed wood).
23) Cover and seal open at the top of the foundation wall where gaps in the block are.
24) Provide a proper metal stem temperature measuring device, Vomit and diarrheal clean-up kit with procedure, employee illness training documentation, PIC and certified manager training certificates.
25) Provide a final building and fire approval and Certificate of Occupancy.
26) A signed application (application to be provided once final approve is provided) and proper license fee is require before operation.
