Observed accumulation of dust, dirt and other debris in the interior of the ice cream bunkers, interior of reach-in freezer, and the basins of the three-compartment sink.

Unnecessary or nonfunctional items and /or litter on premises. Observed paint supplies and equipment and other items that are not used for the operation of the facility.

Physical facilities not maintained in good repair. Observed multiple damaged and stained ceiling tiles throughout the facility.

Light intensity less than ten foot candles in required areas. Obsered measured light intensity in the back of the store at 3-4 foot candles. Observed multiple light bulbs that need changed out and are no longer working.
