3717-1-04.1(KK) / Food equipment - certification and classification.
Equipment is not approved by a recognized testing agency. Observed multiple pieces of non-commercial equipment at time of inspection (toaster, microwave, crockpot, etc.).
To prevent health hazards, food equipment that is acceptable for use in a FSO or RFE shall be approved by a recognized food equipment testing agency.
3717-1-06.4(L) / Removing dead or trapped birds, insects, rodents, and other pests.
Observed the presence of dead pests (mouse) in the basement at time of inspection.
To prevent health hazards, remove all dead pests from the FSO. Continue to monitor for pests with a licensed pest control contractor. Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests.

Currently, there is not an approved preparation sink to properly wash fruits and vegetables. The PIC stated that tomatoes are no longer served and lettuce is purchased pre-washed. No other fruits and vegetables are provided.
