3717-1-03.2(A)(1) / Preventing contamination from hands - hand washing
Observed food employee not washing hands properly at the time of inspection. Employee used 3-compartment sink to rinse hands.
To prevent contamination, food employees shall wash their hands as specified under paragraph (B) of rule 3717-1-02.2 of OAC. Hand washing shall occur at the designated handwashing sink.
3717-1-04.4(A)(3) / Equipment - cutting or piercing parts of can opener not maintained.
Observed can opener not maintained (dirty) at the time of inspection.
To prevent contamination, cutting or piercing parts of can openers shall be kept clean. The can opener shall be cleaned as often as needed to remain clean.
3717-1-06.4(A) / Repairing.
Observed the physical facilities that are not being maintained in good repair. The bathroom exhaust fan was inoperable at the time of inspection.
To prevent health hazards, the physical facilities shall be maintained in good repair. Repair bathroom fan.
3717-1-06.4(B) / Cleaning - frequency and restrictions.
Observed a build-up of grease and debris on the exhaust hood at the time of inspection.
To prevent health hazards, the physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. The exhaust hood shall be cleaned and maintained.

Food is delivered on off hours, ensure that frozen and TCS foods show no signs of temperature abuse when inspecting.
Person-in-charge demonstrated good knowledge in food safety. Facility appeared well maintained at the time of inspection.
A level 2 ODH certification in food safety training is required to be obtained for one operator of the facility by March 1, 2017.
