Repeat Facility does not have an employee with level two certification in food protection.

3717-1-04.3(C) / Fixed equipment installation: elevation or sealing.
Equipment not properly elevated off the floor. Observed shelving units in the dry storage area with the bottom shelf installed on the floor causing the area to be difficult to clean.

3717-1-06.1(L) / Toilet rooms - enclosed.
Toilet room did not have tight-fitting, self-closing door. PIC stated the damage door is going to be replaced and has the new door in the facility waiting to be properly installed.
