Critical Presence of a large population live insects (drain flies) at bar. Education was provided about proper pest control and the PIC stated that a professional pest control contractor was scheduled.

3717-1-06.2(B) / Handwashing cleanser - availability.
No soap at handwashing sinks at bar and kitchen preparation area.

3717-1-06.2(C) / Handwashing sinks - hand drying provision.
No towels or drying device at the preparation area handwashing sink.

3717-1-04.8(A) / Equipment and utensils - air-drying required.
Equipment and utensils are not being air dried. Ware wash beverage containers were stacked together wet without air drying.

3717-1-04.5(A)(3) / Cleanliness of nonfood-contact surfaces of equipment.
Repeat Non-food contact surfaces of equipment are unclean (microwave, cold holding equipment, cooking equipment, ice machine, storage racks and shelving).

3717-1-06.4(A) / Repairing.
Repeat Physical facilities not maintained in good repair. There are a large number of non-working light bulbs in the kitchen. There is caulk missing at the ware washing station. There are multiple damaged ceiling tiles in the kitchen and restroom.

3717-1-06.4(B) / Cleaning - frequency and restrictions.
Repeat Facility not maintained clean. The exhaust hoods are not clean. The floors are not clean in the kitchen and at bar.
