[?] A summary of the violations found during the inspection are listed below.

3717-1-05.1(O)(2) / Using a handwashing sink - other uses prohibited.
Critical Handwashing sink is being used for purposes other than handwashing. Observed a garbage disposal in the hand sink at time of inspection.
To prevent contamination, a handwashing sink may not be used for purposes other than handwashing.
3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Observed a direct connection between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed at time of inspection (ice machine, ice bin, and "preparation sink").
To prevent health hazards, install an air gap on the drains. A direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code for: a warewashing machine between its waste outlet and a floor drain when the machine is located within five feet (1.5 meters) of a trapped floor drain and the machine outlet is connected to the inlet side of a properly vented floor drain trap; or a warewashing and culinary sink.
3717-1-07.1(C) / Poisonous or toxic materials: Conditions of use.
Critical Observed unapproved application or use of a poisonous or toxic substance. A package of rodent poison was placed under the bar at time of inspection.
To prevent health hazards, discontinue the use of rodent poison. Poisonous or toxic materials shall be used and applied according to law and as specified in this rule.

Please incorporate a policy regarding the handout provided for potential vomiting or diarrheal events (cleanup guidelines, etc.).
A handout was issued to the PIC at time of inspection regarding an employee sick policy. Please incorporate this material ASAP.
The PIC is required to obtain Level 2 certification in food protection (certified by ODH) prior to 3/1/17.

There was no evidence of rodents at time of inspection. Future pest control shall be done by a licensed pest control contractor.
