3717-1-05.1(J) / Backflow prevention device - when required.
Critical Observed no approved backflow prevention device on the plumbing system with a hose connection at the time of inspection.
To prevent contamination, a plumbing system shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the FSO or RFE, including on a hose bibb if a hose is attached or on a hose bibb if a hose is not attached and backflow prevention is required by the Ohio building code, by: (1) Providing an air gap as specified under paragraph (D) of this rule; or (2) Installing an approved backflow prevention device as specified under paragraph (E) of this rule. If hose is to be left on the plumbing fixture, provide a ASSE 1011 Backflow prevention device. Recommend providing a shorter hose that does not touch the mop sink bottom.
3717-1-05.3(C) / Backflow prevention.
Critical Repeat Observed a direct connection between the sewage system and a drain originating from equipment (dipper well and Ice bin) in which food, portable equipment, or utensils are placed at the time of inspection.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment in which food, portable equipment, or utensils are placed unless allowed by the Ohio building code for: a warewashing machine between its waste outlet and a floor drain when the machine is located within five feet (1.5 meters) of a trapped floor drain and the machine outlet is connected to the inlet side of a properly vented floor drain trap; or a warewashing and culinary sink. Provide a proper air gap on the dipper well and ice bin.

3717-1-02.2(G) / Jewelry - prohibition.
Corrected During Inspection Observed food employees wearing jewelry on arms or hands during food preparation at the time of inspection.
To prevent contamination, except as provided by (G)(2) and (G)(3) of this rule, food employees may not wear jewelry on their arms or hands.
3717-1-03.2(M) / Wiping cloths - use limitation.
Repeat Corrected During Inspection Observed improper use and/or maintenance of wiping cloths. Observed soiled wiping cloths were being left out on counters at the time of inspection.
To prevent the growth of harmful organisms, cloths that are in use for wiping food spills from tableware and carry-out containers shall be maintained dry and used for no other purpose; cloths in use for wiping counters and other equipment surfaces shall be held between uses in a chemical sanitizer, or laundered daily; cloths used with raw animal foods shall be kept separate from other cloths, dry cloths and solutions shall be free of food debris and visible soil; containers of sanitizer solutions shall be stored off the floor, and used in a manner that prevents contamination. Single-service articles, single-use articles or single-use disposable sanitizer wipes shall be used in accordance with EPA - approved manufacturer's label use instructions.
3717-1-04.4(A)(1) / Equipment - good repair and proper adjustment.
Repeat Observed handled and door seal on the tall double door reach-in cooler to be damaged at the time of inspection. Equipment and/or components are not maintained in good working order.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. To maintain equipment in proper working order, repair the damaged door handles and door seals of equipment when needed.
