Comments
This pre-licensing inspection was completed per the request of the owner to complete the Facility Review. The following shall be completed before a license may be issued:
1) Finish installing the lighting in the outdoor dry storage shed.
2) Finish caulking along all floor and wall junctures. This includes around the floor and wall junctures of the black cabinets that house the reach coolers behind the bar. Along the base of the pizza over, shelving under bar, around the restroom privacy posts and in the server area. Seal hole in wall behind server coffee station.
3) Provide additional light over the 3 compartment sinks and hand sinks of the bar. Shall be 20 foot candles.
4) Provide proper thresholds and weather stripping at each door way to the outside.
5) Provide a final menu. Recommend allowing the health department to review before menu is sent to the printers.
6) Provide proper dunnage racks in the walk-in cooler and freezer.
7) Provide a vomit and diarrheal clean up kit with acceptable procedure.
8) Provide verification that staff has been training on the illnesses and symptoms to report to managers. See Attachment to inspection email.
9) Review sections 8.5 of the Ohio Food Code regarding dogs on the patio. Provide all required signage and supplies per the code.
10) Provide a signed application and license fee for the amount of $640 for license JSMH-BHVJGH.
11) Provide a official certificate of Occupancy once issued.
