3717-1-03.2(C) / Packaged and unpackaged food - preventing contamination by separation, packaging, and segregation
Critical Observed frozen chicken (repackaged) that was being stored above ice buckets at the time of inspection.
To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule. Discontinue storing frozen chicken above ice buckets. The ice buckets shall be properly warewashed before use.

3717-1-04.2(H)(2) / Temperature measuring devices - mechanical warewashing.
Repeat Observed an irreversible registering temperature indicator (thermometer or labels) was not readily accessible to test and montior hot water mechanical warewashing equipment at the time of inspection.
To ensure ware washing equipment is properly sanitizing, provide a irreversible registering temperature indicator (thermometer or labels). A temperature measuring device is essential to monitor mechanical ware washing to ensure sanitization.
3717-1-04.8(F) / Storage - prohibitions.
Observed cleaned and sanitized equipment, utensils, or single-use articles were stored in a prohibited area. Observed plastic cups beside soda dispensing equipment were exposed, paper/plastic cups and plate in the dry storage areas were unprotected, plates were unprotected under staircase, and multiple serving bowls were not being stored inverted in dry storage areas at the time of inspection.
To prevent contamination, equipment, utensils, laundered linens, or single-service articles or single-use articles may not be stored in the areas specified under this rule where contamination might occur. Discontinue storing the items that are listed above unprotected.
3717-1-05.1(S)(2) / Plumbing system - maintained in good repair.
Observed the plumbing system under the 3-compartment (dairy side) was not properly maintained and leaking at the time of inspection.
To prevent health hazards, a plumbing system shall be properly maintained. Repair plumbing system to be in good working order.
3717-1-06.2(I)(3) / Lighting - intensity (50)
Repeat Observed the measured light intensity at the dirty dish section of the main warewashing machine where an employee works with food, utensils or equipment was less than fifty foot candles at the time of inspection.
For safety, the light intensity shall be at least fifty foot candles (five hundred forty lux) at a surface where a food employee is working with food, utensils or equipment such as knives, slicers, grinders, or saws where employee safety is a factor. Provide proper lighting to meet the requirements of this rule
