3717-1-05.3(C) / Sewage and other liquid waste: Backflow prevention.
Critical Repeat Observed a direct connection between the sewage system and a drain originating from equipment (warewashing sink used to wash fruits and vegetables) at the time of inspection.
To prevent health hazards, a direct connection may not exist between the sewage system and a drain originating from equipment (warewashing sink) in which food, portable equipment, or utensils are placed. Install a culinary sink that has a proper air gap or install an air gap for the 3 compartment sink.

3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Observed equipment (rubbermaid containers) that were not approved by a recognized testing agency at the time of inspection.
To ensure proper warewashing, food equipment that is acceptable for use in a FSO shall be approved by a recognized food equipment testing agency equipment. Commercial containers that are approved by a recognized testing agency shall be used.
3717-1-04.4(Q) / Utensils, temperature and pressure measuring devices - good repair and calibration.
Observed thermometers that were not kept in good repair or calibration at the time of inspection.
To prevent health hazards, food temperature measuring devices shall be calibrated as necessary to ensure their accuracy. Provide a properly calibrated thermometer to meet the requirements of this rule.

Opened milk is considered a TCS food and shall be properly date marked if being held for more than 24 hours.
A level 2 Ohio Department Health certification in food safety is required to be obtained for one operator of the facility by March 1, 2017.
