[?] A summary of the violations found during the inspection are listed below.

3717-1-03.2(M) / Wiping cloths - use limitation.
Corrected During Inspection Observed improper use and/or maintenance of wiping cloths at the time of inspection. Wet wiping cloths were being left out on counters.
To prevent contamination, (1) Cloths that are in use for wiping food spills from tableware and carry-out container that occur as food is being served shall be maintained dry and used for no other purpose; (2) Cloths that are in use for wiping counters and other equipment surfaces shall be: (a) held between uses in a chemical sanitizer of the appropriate concentration; and (b) shall be laundered daily. (4) Dry wiping cloths and the chemical sanitizing solution in which wet wiping cloths are held between uses shall be free of food debris and visible soil; (5) Containers of chemical sanitizing solutions specified in paragraph (M)(2)(a) of this rule in which wet wiping cloths are held between uses shall be stored off the floor and used in a manner that prevents contamination of food, equipment, utensils, linens, single-service articles, or single-use articles. Discontinue leaving sanitizer wiping cloths out on the counter.
3717-1-04.2(I) / Sanitizing solutions - testing devices.
Observed no chlorine test kit available for measuring the concentration of the sanitizer (bleach) at the time of inspection.
To ensure proper sanitization, a test kit or other device that accurately measures the concentration in PPM (mg/L) of sanitizing solutions shall be provided. Provide a chlorine test kit.
3717-1-04.5(D) / Nonfood-contact surfaces - cleaning frequency.
Observed accumulation of soil residue on nonfood-contact surfaces on multiple lids of the dry powder containers at the time of inspection.
To prevent contamination, nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. The lids for the powder containers shall be cleaned and maintained.
3717-1-04.8(E) / Equipment, utensils, linens, single-service articles, and single-use articles - storage.
Observed single-service and single-use articles were not properly stored at the time of inspection. Multiple items were being stored on the floor.
To prevent contamination, single-service articles, and single-use articles shall be properly stored (a) In a clean, dry location; (b) Where they are not exposed to splash, dust, or other contamination; and (c) At least six inches (fifteen centimeters) above the floor unless the items are kept in closed packages on dollies, pallets, racks, and skids that are designed as specified under paragraph (II) of rule 3717-1-04.1 of this code. Discontinue storing items on floor.

Per discussion with the owner, a commercial refrigerator was purchased and being shipped. The Health Department will need the make and model of that unit.
Please provide documentation that the syrups are safe to not refrigerate once it is opened.
