3717-1-04.1(KK) / Food equipment - certification and classification.
Repeat Observed equipment (microwave) that was not approved by a recognized testing agency at the time of inspection.
To prevent health hazards, food equipment that is acceptable for use in a FSO shall be approved by a recognized food equipment testing agency. Replace with a commercial microwave as soon as possible.
3717-1-06.1(I) / Light bulbs - protective shielding.
Observed light bulb in the dry storage area that was not properly shielded or coated where required at the time of inspection.
To prevent health hazards, light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles and does not apply in areas used only for storing food in unopened packages. Provide proper shielding for the light bulb in the dry storage area.

Facility appeared clean and well maintained at the time of inspection.
Employee food and other materials shall continue to be kept separate from the operation.
Floors, walls, and ceilings shall be smooth and easily cleanable, if they no longer meet that requirement then they shall be replaced.

As discussed the employee health reporting policy shall be in written form and available for review.
A diarrheal and vomit clean-up procedure shall be posted as a written document.
A level 2 ODH certification in food safety is required to be obtained for one operator of the facility by March 1, 2017.
