| In order for a RFE risk class III license to be issued the following must be completed: 1) Seal all the holes and opens in all the walk-in coolers. Seal the areas that has the spray foam exposed. 2) Since the emergency fire doors located at the receiving dock are planned to be used a vendor entrance, the doors are required to be tight fitting, self-closing and self latching. 3) New cold cut cold holding equipment shall be up and running holding at 41 degrees F or below. 4) Provide a ASSE 1011 on all hose bibb connections. Only one was observed in the on the ware washing machine pre-rinse sprayer. 5) Provide a internal temperature measuring device in all cold holding equipment. 6) Provide an alarm on the ware washing machine to indicate when the detergent is out. (visual or audible) 7) Reconnect the detergent line that was damaged. 8) Provide a final building and fire inspection. A Certificate of Occupancy will need to be provided. 9) Submit a signed application and license fee for the amount of $973.00 for License number JSMH-9M2KLM. This is not a final list and is subject to change upon additional pre-licensing inspection. |
